Job DescriptionReporting to the Director of Care Management, the Manager for Care Management Operations, (is responsible for supporting the Director in driving Summit Health’s care management strategic priorities and performance management outcomes. In this role, the Manager serves as a critical interface between care management leadership and other key stakeholders that help support care management, population health and value-based care functions. As delegated by the Director, the Manager will manage initiatives as assigned, including lead, monitor, and manage progress of key strategic initiatives and any projects that cross interdepartmental collaborations. The Manager promotes an environment of teamwork and collaboration while demonstrating effective change management strategies that produce cost effective and innovative outcomes.Essential Job Functions:1. In collaboration with the Director of Care Management and leadership to develop methodologies and infrastructure to measure the impact of executed initiatives.2. Synthesize information into impactful materials and presentations to facilitate strategy recommendations to leadership. 3. Effectively work in a matrix environment to accomplish work across multiple teams across internal and external constituencies such as health plans/payers.4. Identifies opportunities to improve existing operational policies and procedures and develops new policies and procedures as necessary.5. Identifies, communicates, and escalates issues to the Director of Care Management on a timely basis. Independently problem solves programmatic issues and implements appropriate solutions.6. Collaborates with care management leaders to ensure all policies, procedures and workflows meet continued regulatory and contractual compliance.7. Works with Finance to monitor performance and value-based care revenue.8. Develops and oversees the production of standard reports to monitor and report on overall metrics and care management program evaluation.9. Accountable for tracking KPI metrics for care management goals and outcome targets where applicable and implementing operational changes in partnership with managers of care management as they relate to department metrics and clinical program performance.10. Accountable for timely notification to the Director, and manager(s) if performance falls below expectations. Bachelor’s degree in Health Care Administration, Public Health, Nursing, or related field.2. Master’s degree in Health-Related/Public Health field preferred.3.
